Qualifications:

Must have a minimum of seven Full Time Employees whose average pay is equal to or greater than $30,000 and the business must offer medical benefits where the employee pays no more than 50% of premiums.
The business must have annual revenues of more than $1 million.

Last 12 months profit and loss statement
Current balance sheet
Oklahoma Tax Commission Report (Sales Tax Paid Receipt), if applicable
IRS Form W9, signed and dated
Quotes for equipment and installation, if applicable
