Welcome to IHI’s Safer Together: A National Action Plan to Advance Patient Safety Self-Assessment Tool!

We’re excited to support you in evaluating and advancing your organization’s patient safety practices.

To get started, please follow these steps as the Self-Assessment Tool Administrator (defined as the person who will identify additional team members and receive the final report once the Self-Assessment Tool is completed):

Please note that the 14-calendar-day completion window starts as soon as you enter your information below. Completing Step 1 before initiating this process is essential.

  1. Prepare Your Team Details Before You Initiate the Self-Assessment Process:
    • Before proceeding, ensure you have first gained senior leader agreement for the interdisciplinary team members you want to include. These individuals should be familiar with some or all aspects of your organization’s safety strategy. Team members might include C-suite leaders, safety/quality/risk leaders, chairs of key committees (e.g., governing body, quality/safety committee, Patient and Family Advisory Committee), and others who can offer informed perspectives.
    • Collect their contact information, including:
      • Name
      • Title
      • E-mail Address
    • Select the best timing for when you want to start the Self-Assessment Tool. You might consider competing priorities, avoiding vacations and holidays, etc. Once you input your information below, the team members you identify on the next page will have 14 calendar days within which to complete it.
    • Communicate to your selected team regarding the purpose and rationale for completing the Self-Assessment Tool. Noting the 14 calendar day window timing, advise them that they will receive an invitation e-mail with a unique Self-Assessment Tool link from noreply@brilliantassessments.com. Let them know the assessment typically takes 30 minutes to 1 hour to complete, so they can plan enough time to provide thoughtful scores and optional comments.
  2. Initiate the Self-Assessment Tool Link:
    • Begin by entering your contact information with the requested information in the fields provided below.
    • Enter your team members' details on the next page through the Add Respondent button.
    • When you have added all members of your team, automatically send invitation emails (which include the unique Self-Assessment Tool link for your organization) by selecting the Send Invitations button.
    • An additional email will be sent to you with a link to directly access the page to add more team members later if desired.

Use of Data

  • Organizational Reports:

The data provided by you or your assigned team members (Respondents) will be utilized to generate a comprehensive report assessing your organization's safety readiness across the four foundational areas outlined in Safer Together: A National Action Plan to Advance Patient Safety.

The data will be presented in a deidentified manner to support psychological safety among your team.

  • Maintenance of Data: 

All collected data will be anonymized by IHI and may be aggregated with data from other organizations to identify trends and highlight opportunities for improvement in patient and workforce safety.

  • Individual organizations, facilities, systems, or specific responses will never be identified by name or location in any reports or analyses.

Please note:

Please check with your Information Systems/Technology department to ensure that e-mails sent from noreply@brilliantassessments.com are identified as "safe sender". This is important to prevent the invite emails from either being blocked or caught up in spam/junk mail.
